The Opportunity
This role offers the chance to work alongside experienced PM&R physicians in a highly collaborative, interdisciplinary environment. It's well-suited for a clinician who wants to focus on functional recovery, continuity of care, and meaningful patient outcomes--while maintaining a flexible schedule and no on-call responsibilities.
You'll play a central role in helping patients progress through rehabilitation while working with a team that prioritizes coordination, consistency, and measurable impact.
What You'll Do
- Partner with a PM&R physician to manage patient care in skilled nursing and sub-acute rehab settings
- Collaborate with therapy teams (PT/OT/ST) to align medical treatment with functional recovery goals
- Coordinate care with primary care providers, nursing staff, social workers, and families
- Manage pain and rehabilitation needs with a focus on improving function and reducing hospital readmissions
- Ensure timely, accurate documentation, with flexibility to complete charting remotely

What We're Looking For
- Licensed Nurse Practitioner or Physician Assistant (required)
- Active certification as an NP or PA (required)
- Current Michigan license (required)
- Strong communication and collaboration skills in a team-based care environment
- Ability to work independently across multiple facilities
- Reliable transportation
